We will be asking for information regarding your income (before and after taxes); cost of living (rent/mortgage, utilities, groceries, insurances, etc); and debts, both secured (such as mortgage
and car loans) and unsecured (personal loans, credit cards, lines of credit).

To help you fill out the form we recommend that you have your most recent bills/creditor
statements on hand. If you do not have statements please enter the approximate balances.  
When filling out the income and living expenses sections please enter monthly amounts.
For example, if you spend $600.00 each year on clothing, enter $50.00 ($600.00/ twelve
months). If you are unsure of some of your expenses, enter an approximate amount or
simply leave it blank until you speak with your counsellor.

Upon receipt of your application we will contact you within two business days to schedule an appointment where one of our counsellors will review and discuss the information with you. 
Once the appointment is complete the counsellor will provide you with a written assessment
along with a list of recommendations to help you address your financial concerns.
